Filing Status: Select from the pull-down list.
Allowances: Enter the number of exemptions this employee is claiming.
Additional Withholding: Enter a dollar amount, if this employee wants more withheld than would be withheld under the number of allowances claimed.
Earned Income Credit: Select No. The Advanced Earned Income Tax Credit was repealed on 8/10/2010.
W-5s Filed: Necessary only when Earned Income Credit is Yes.
FICA Exempt: Select Yes if the employee should not have FICA withheld from his/her wages. Select No to withhold FICA from the wages.
Medicare Exempt: Select Yes if the employee should not have Medicare withheld from his/her wages. Select No to withhold Medicare from the wages.
GTL Coverage Type: Select from Fixed, Multiple of actual pay, Multiple of job salary.
If Fixed:
GTL Coverage: Enter the amount of the GTL coverage.
GTL Effective Date: Enter the effective date.
Distribution: How should the GTL benefit be distributed? Select from Evenly over Pay or Lump Sum.
If Multiple of actual pay or Multiple of job salary:
GTL Factor: Enter the number by which the actual pay or job salary will be multiplied.
GTL Additional Amount: Enter the amount of the additional GTL policy, if applicable.
GTL Effective Date: Enter the effective date.
Complete the State Withholding Information. The fields displayed will vary according to state.
